Preferably using a ballpoint pen, fill in either the blue or yellow part of the Traffic Announcement (For example, a French driver can fill out his part of the application form in French, leaving the unfilled part of the Notification of Accident that you will fill in in your language). You will understand the meaning of the questions, if you compare it with your document).

Please, keep in mind:

a. Clearly note (in paragraph 10) the location of the initial car hit;

b. Put the cross (X) in each corresponding cell on your side (paragraph 12)

Write the total number of cells marked with a cross.

c. Draw a diagram of the car accident (paragraph 13), indicating all the necessary information.

When you are convinced of the accuracy of the facts presented, please, sign the Notice and obtain the signature of the other driver (paragraph 15).

Take a copy of the completed form of the accident notification. (Or take a photo of it)
